I say 1. “lower your voice. You don’t want the people outside to hear, do you?” 2. “lower your voice. You don’t want the people out there to hear, do you?” Which one sounds natural?

Both sound natural but they also both sound like you're talking about something private you don't want overhead rather than just wanting someone to be quieter.

Outside is nonspecific. It's a pretty large concept, encompassing all the known universe. Out there is a bit more specific. "There" is demonstrative, so it points out a specific place in the speaker's mind. edit: both are completely fine and interchangeable unless you're being really picky
